In my work at Ghetto Outer Borough Hospital (heretofore named GOB Hospital), I look at a lot of patient charts.
In the basic info section, there’s a label called “Appearance.” Usually providers will write something like:
APPEARANCE: pleasant, alert, oriented.
Yesterday, I stumbled across a resident who had written:
APPEARANCE: Pt wearing sunglasses, multiple necklaces, multiple rings, and bracelets.
